Annual Family and Community Survey

We want to hear from you!

Tell us how we’re doing in our mission to “inspire and empower each individual to learn, grow and excel.” Results will be used to evaluate our work and provide feedback for reflection and improvement.

Survey Directions: Please complete this survey by selecting the response that is most appropriate for you. If you are a parent/guardian, please complete this survey for each school in which you have students enrolled; you will have the opportunity to select another school after you respond to the initial school you selected. Please complete this survey by Friday, April 23.

Thank you for your time. Your feedback really matters to us!
